Looking for your next opportunity within RFQ? Join our Hospital to Home team in Toowoomba and support people as they take important steps from hospital back into community life.

As a Recovery Support Worker, you’ll partner with individuals to build confidence, reconnect with what matters to them and draw on their strengths and resilience as they move forward in the community.

Working from a strengths based, recovery oriented approach, you’ll support people to navigate the transition back into everyday life, strengthen personal and community connections and access opportunities that support wellbeing, independence and inclusion. Through genuine relationships and collaborative practice, you’ll help people identify their goals, build on their capabilities and create a hopeful, self directed pathway forward.

This is an opportunity to bring your existing RFQ knowledge and experience into the Hospital to Home team while continuing to grow your practice, build new connections and contribute to meaningful outcomes for the people we support.

Your Key Responsibilities

- Support people transitioning from hospital back into community life
- Build positive relationships that promote confidence, wellbeing and independence
- Assist individuals to identify goals and take steps towards achieving them
- Support community participation and connection with services and natural supports
- Engage with families, health professionals and community stakeholders to support positive outcomes
- Deliver person centred support that reflects individual needs and aspirations
- Maintain accurate records using digital systems and mobile technologies
- Participate in team meetings, supervision and ongoing professional development

About You

You are passionate about supporting people on their recovery journey and believe in the value of strengths based,



person centred practice. You bring:

- A degree in a human services related field with at least one year of relevant experience, or less formal qualifications with substantial relevant experience
- Demonstrated experience supporting people living with mental ill health
- Strong relationship building skills and the ability to engage effectively with clients, families, health professionals, justice services and community stakeholders
- Knowledge of recovery oriented practice and community reintegration supports
- A strengths focused mindset and genuine commitment to supporting people to achieve their goals
- Strong communication, documentation and solution focused problem solving skills
- Cultural responsiveness and the ability to work effectively with people from diverse backgrounds
- Adaptability, resilience and optimism when supporting people through change and transition
- A strong sense of professional boundaries, integrity and accountability
- Confidence using digital systems and mobile technologies

The Opportunity

This role offers the opportunity to broaden your experience within RFQ and contribute to a service supporting people through an important point in their recovery journey.

The position offers:

- $80,000 - $85,000 base annual salary, depending on experience, plus superannuation
- Salary packaging benefits of up to $15,900 for general living expenses and an additional $2,650 for entertainment expenses, including access to novated leasing
