Medical Scientists, Pharmacists and Psychologists Victorian Public Sector (Single Interest Employers) Enterprise Agreement 2021-2025

YDMH is excited to announce we are seeking an On-Site Aged Care Pharmacist for a Fixed Term role from 19/10/2026 to 19/10/2027.

This role will work collaboratively with residents, families, nursing staff and medical practitioners to improve health outcomes of residents, supporting safe effective care.

Key Responsibilities

- Person centred care – collaborating and communicating effectively with residents, family and carers to understand resident preferences, beliefs and priorities.
- Collaboration and communication – develop an interprofessional working relations, integrating with the multidisciplinary teams, both internally and externally.
- Medication Management – Provide expertise in medication management, reducing medication related harm and optimisation of medications.




- Quality Improvement – facilitate quality improvement activities, including analysing, reporting of results and recommendations.
- Clinical Governance – this role will contribute to RACF clinical governance and encourage a culture of continuous quality improvement, ensuring secure and quality care for residents.
