We are seeking an experienced cardiac nurse to support our dynamic virtual health service for members located in Australia and New Zealand.

Teladoc Health’s Heart Coach Service is designed to help people recover from and prevent cardiac events. Delivered by a cardiac nurse, the virtual cardiac preventative service is tailored to individual member needs, providing a personalised, engaging and motivating user experience for participants, from the comfort of their own homes. Members are then provided support to share the outcome of their assessment, including any treatment recommendations, with their own GP and treating team.

What you'll be doing The key functions of this role include:

- Educating patients about cardiovascular health, risk factors, lifestyle modifications, and self-management techniques, empowering them to take control of their health.
- Embedding health behaviour change principles into program delivery,



facilitating participants' adoption of healthier lifestyles and self-management practices.
- Delivering patient-centered care aimed at minimising preventable hospital admissions and promoting overall well-being.
- Detecting and addressing medical and other issues that arise during the program, referring patients to appropriate Teladoc Health services and/or healthcare providers as needed.
- Providing continuity of care for patients transitioning into the program following discharge from the hospital, ensuring seamless integration into the program's framework.

To be successful in this role You will be an experienced, registered practicing cardiac nurse who is comfortable operating in a virtual environment. In addition, you'll have:

- A current registration with AHPRA as a registered nurse.
- Experience in cardiac care, cardiac rehabilitation, or working with clients with chronic health conditions, including cardiovascular disease and diabetes.
- Knowledge of and commitment to best practices in chronic disease management, emphasising prevention, healthy lifestyle promotion, and client-directed goal setting.
- Excellent communication, interpersonal, and teamwork abilities.
- Commitment to ongoing professional development and quality improvement initiatives.

Benefits A competitive hourly rate between $60-67 AUD is provided based upon hours worked and services provided. Whilst services are available between 8am to 8pm AEDT, we provide flexibility, and you can choose when and how many hours you wish to work.
