Role Description The Care Partner is a full time, on-site role based in Sutherland, NSW, providing direct in-home support to clients in the local community. Day-to-day tasks include assisting clients with personal care, mobility, and activities of daily living, while promoting independence and safety. The role involves providing companionship, supporting social engagement, and helping with light domestic duties such as meal preparation and household tasks.
Care Partners follow individual care plans, record observations, and communicate changes in client needs to the care team to ensure consistent,
high-quality service. The position requires working collaboratively with clients, families, and health professionals, respecting diverse backgrounds and maintaining professional boundaries at all times.
Qualifications
- Demonstrated experience in person-centered care, including personal care, mobility support, and assistance with activities of daily living.
- Skills in providing companionship, emotional support, and encouraging social engagement for clients with varying care needs.
- Ability to perform light domestic duties such as meal preparation, housekeeping, and basic home safety checks.
- Strong communication and interpersonal skills, including clear documentation and effective collaboration with clients, families, and care teams.
- Relevant care or community services qualification (e.g., Certificate III in Individual Support, Aged Care, or Disability) or progress toward such qualifications.
- Experience or a strong interest in aged care, dementia support, disability support, or post-operative care in home or community settings.
- Current First Aid and CPR certification, valid driver’s license, and reliable transport to visit clients in the Sutherland Shire area.
- Ability to work shifts across weekdays and weekends as needed, with a commitment to reliability, respect, and maintaining client confidentiality.
