Enrolled Nurse

Synergy Community Services is seeking a motivated and compassionate Enrolled Nurse (EN) to join our NDIS team.

This is an exciting opportunity for a Graduate EN who wants to combine hands-on nursing care with clinical leadership, supporting NDIS participants while leading and mentoring our Support Workers.

About the Role

You will provide high-quality, person-centred nursing services to NDIS participants and provide clinical guidance and leadership to Support Workers.

You will help ensure participants receive safe, consistent and individualised support in line with their NDIS goals, support plans and clinical needs.

Key Responsibilities

- Provide nursing care to NDIS participants in community and home-based settings.
- Monitor participants' health, wellbeing and changing support needs.
- Support medication management and clinical interventions within your scope of practice.
- Provide clinical guidance, mentoring and supervision to Support Workers.
- Ensure Support Workers understand and follow participant care and support plans.
- Support the delivery of complex and high-intensity supports where required.
- Identify, document and escalate health, safety and clinical risks.
- Contribute to incident management, clinical reviews and continuous improvement.
- Maintain accurate clinical notes, reports and participant documentation.
- Liaise with GPs, allied health professionals, families, guardians and other stakeholders.
- Promote participant choice,



dignity, independence, privacy and NDIS goals.
- Support a strong culture of quality, safeguarding, accountability and person-centred practice.

About You

We are looking for an EN who has:

- Current AHPRA registration as an Enrolled Nurse.
- Nursing experience in disability, NDIS, community or complex care.
- Strong clinical assessment and documentation skills.
- Experience or confidence in leading and mentoring Support Workers.
- Understanding of NDIS practice standards, participant rights and safeguarding.
- Excellent communication and interpersonal skills.
- A professional, reliable and person-centred approach.
- Ability to work independently and collaboratively.

NDIS Worker Screening Check, First Aid/CPR and Australian driver's licence are highly regarded/required as applicable.
