Are you highly organised, detail-oriented, and passionate about supporting programs that make a real difference in community health?

AccessHC is seeking an enthusiastic Program Support & Data Officer to join our Care Navigators Program . Working closely with the Care Navigator Program Manager, you will play a pivotal role in supporting program operations, reporting, contract administration, stakeholder engagement, and data management to ensure the successful delivery of this important EMPHN-funded initiative.

This role is ideal for someone with experience in project or program administration who enjoys working collaboratively across teams, managing competing priorities, and using data to support quality outcomes.



You will contribute to the ongoing development, implementation, and evaluation of the Care Navigators Program while building solid relationships with partner agencies and key stakeholders.

What you'll bring

- Experience in project administration, program support, or contract administration
- Strong analytical, reporting, and organisational skills
- Excellent communication and stakeholder engagement abilities
- Advanced Microsoft Office skills and a high level of attention to detail
- Experience using Fixus, TrakCare or other electronic health records/client management systems (desirable)
