The Role The Justice Health and Forensic Mental Health Network (the Network) Integrated Care Service (ICS) supports the existing systems to provide and integrated model of care for the patient journey from reception, through custody and into the community. The ICS Care Coordinator position is responsible for the coordination of care for adult patients with chronic and/or complex health conditions. The position provides a support network to the Network clinicians to assist with the continuity of care, case management,
transfer of care into the community and post release follow up as required for custodial patients.
