Are you a compassionate and experienced Registered Nurse looking to make a real difference in the lives of children, young people and older adults living with complex disabilities and health needs.

We are seeking Registered Nurse to join our team at in Yeerongpilly, as well as across various community locations throughout Brisbane.

In this rewarding role, you will work alongside clinical and support teams to promote the health, wellbeing, independence and quality of life of people with complex disability and healthcare needs. You will provide hands-on clinical care, develop individualised health plans, support families and carers, and lead teams to deliver safe, evidence-based and person-centred care.

While supporting children and young people with complex disabilities, as well as older adults with complex health and care needs.

About The Role

Your responsibilities will include

- Provide high-quality, person-centred clinical care to children, young people and adults with complex health and disability needs.
- Assess individual health needs and develop, implement and review tailored health and care plans.
- Support individuals and families with health education, health literacy, risk management and strategies to prevent avoidable hospital admissions.
- Work collaboratively with individuals, families, carers, health professionals and internal teams to promote safe, inclusive community living.
- Provide clinical leadership, mentoring, coaching and training to support workers, carers and team members in complex care practices.
- Apply evidence-based practice, respond effectively to changing needs, work across community settings and participate in a predominantly phone-based on-call roster.
- Provide comprehensive health and continence assessments, while performing a range of nursing duties, including wound care and catheter care,



with a holistic approach to supporting our aged care clients.

Essential Requirements

- Bachelor of Nursing with current AHPRA registration.
- A minimum of 3 years' post-graduate nursing experience, preferably in medical nursing, paediatrics, community nursing, hospital or aged care.
- Current Australian driver's licence and access to a reliable and appropriately insured vehicle.
- Ability to meet suitability requirements, including a satisfactory National Police Check and NDIS Worker Screening Check.
- Ability to travel across Brisbane Metro as required.
- Availability to participate in on-call arrangements.
- Australian citizenship, permanent residency or other suitable Australian work rights.

Desired Requirements

- Postgraduate study in paediatric nursing, community nursing or a related field.
- Experience working within disability, community or complex care environments.
