Remuneration: $41.22 - $57.88 per hour

Where you'll be working

Join the Drug Health Assertive Community Management Team at Mt Druitt Centre for Addiction Medicine in this exciting district-wide opportunity. You will work alongside a dedicated multidisciplinary team supporting people experiencing complex health and substance use challenges in the community.

- Be part of a specialised Drug Health service supporting vulnerable and diverse communities
- Work collaboratively with nurses, clinicians and other health professionals
- Contribute to a culture of respect, open communication and shared problem solving
- Support people and their carers through compassionate, culturally appropriate care
- Help drive continuous improvement and evidence-based practice across the service

What You'll Be Doing As a Registered Nurse, you will provide safe, person-centred and evidence-informed nursing care while delivering assertive community-based Drug & Alcohol case management.

- Assess, plan, implement and evaluate individualised nursing care
- Provide comprehensive case management to people living in the community
- Build therapeutic relationships with patients, carers and the multidisciplinary team
- Maintain accurate clinical documentation, care plans, progress notes and referrals
- Support the education and development of nursing staff and contribute to quality improvement

Essential Criteria

- Current Nursing Registration (RN) with the Nursing and Midwifery Board of Australia (AHPRA)

Selection Criteria

- Demonstrated knowledge of drug and alcohol dependence issues and treatment interventions.
- Immunisation Certificate from the College of Nursing or willingness to attain same
- Current unrestricted NSW Drivers' license and willingnss to use WSLHD vehicle when necessary
- Demonstrated effective interpersonal skills, excellent written and verbal communication skills within a multidisciplinary team
- Evidence of participation in own professional development and commitment to maintaining competence and enhancement of skills, and willingness to apply and share professional expertise with colleagues
- Evidence of computer literacy and understanding of relevant health related information systems
- Demonstrated commitment to person centred care and NSW Health Core Values
