Geraldton Regional Aboriginal Medical Service (GRAMS) is seeking a passionate Aboriginal Health Practitioner (AHP) to join our team.
n
Working within a multidisciplinary team, you will provide culturally protected primary health care, health education and ongoing support to Aboriginal clients both in the clinic and the community.
n
About the Role
n
As an Aboriginal Health Practitioner, you will:
n
n
Deliver culturally appropriate primary health care services.
n
Conduct health assessments and provide health education.
n
Support chronic disease management and preventative health programs.
n
Work collaboratively with GPs, nurses and visiting specialists.
n
Promote health and wellbeing through community engagement.
n
Participate in continuous quality improvement and health promotion initiatives.
n
Current registration as an Aboriginal Health Practitioner with AHPRA.
n
Demonstrated understanding of Aboriginal health and cultural issues.
n
Strong communication and interpersonal skills.
n
Ability to work independently and within a multidisciplinary team.
n
Current unrestricted driver's licence.
n
National Police Clearance and Working with Children Check (or ability to obtain).
n
