ABOUT THE ROLE
The Support Worker provides in-home and community-based supports to NDIS participants and TAC clients to enable them to maintain and/or increase their independence, safety, participation, and wellbeing.
The role is delivered in a person-centred, rights-based manner, consistent with the NDIS Practice Standards, NDIS Code of Conduct, TAC requirements, and West Wimmera Health Service (WWHS) policies and values.
KEY SELECTION CRITERIA
Essential
Demonstrated effective communication and organisational skills.
Commitment to person‐centred practice, empowerment, inclusion, and participant choice and control.
Ability to work independently and as part of a multidisciplinary team.
Current First Aid Certificate (including CPR), or willingness to obtain.
Desirable
Certificate III or higher in:
Individual Support
Disability
Aged Care
Community Services
or a related field
Previous experience supporting people with disability, injury, or ageing in community or residential settings.
Experience working within NDS and/or TAC funded services.
PRE-REQUISITES
NDIS Worker Screening Check.
Working with Children Check.
Evidence of required vaccinations (including influenza) as per Department of Health directives.
Current Australian Driver's Licence.
Ability to travel between client locations.
Able to apply First Aid.
KEY RESPONSBILITIES
Provide support in accordance with each client or participant's Care Plan, support plan, and funded supports.
Support people to exercise choice and control, maintain dignity, and work toward their individual goals.
Assist clients/participants to live safely and meaningfully within their home and community environments.
Provide supports consistent with TAC-approved services and rehabilitation goals.
Observe and report changes relevant to accident-related injuries or functional capacity.
Ensure accurate reporting and documentation to support TAC requirements and funding accountability.
Escalate matters related to TAC clients to the TAC Program Lead or NDIS/TAC Coordinator as appropriate.
