Northern Health are seeking applications from motivated and suitably experienced people to join the Patient Watch Program.
Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne's rapidly growing outer north.
Services are provided through our six main campuses:
Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn and Mernda Community Hospitals Centres and Kilmore District Hospital. In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park, Mernda and Coburg.
About the Role:
The role of Tele-navigator requires an enthusiastic and empathetic person to support vulnerable people to remain healthy at home, by completing daily self-rated health checks via telephone. The Tele-Navigators provide practical and culturally sensitive interactions as a consistent point of contact for patients,
and work in a team with clinical staff members (Health coaches).
To be successful in this role you will have:
- Robust interpersonal skills with a positive, proactive approach
- Demonstrated strong interpersonal skills with a positive, proactive approach
- Well-developed organisational skills, and the ability to work independently and as a member of a team
- An understanding of and commitment to a holistic approach to vulnerable patients, especially those with complex chronic health issues
- Sound telephone and computer skills including keyboarding and the use of the Internet and smartphone applications
