About the Role

n

As a Support Worker, you will provide individualized, person-centred support to adults, young people, and children with disability and complex support needs.

n

You will work alongside participants, families, carers, and community networks to support individuals to achieve their goals while promoting dignity, independence, choice, and inclusion.

n

We're looking for compassionate, reliable, and values-driven Support Workers to join our team in Cragmuir, VIC

Key Responsibilities

- Support participants with daily living activities and personal care
n
- Assist with household and domestic tasks
n
- Encourage independence and skill development
n
- Support community participation and social inclusion
n
- Implement individual support plans and goals
n
- Follow Positive Behaviour Support Plans (BSPs)
n
- Administer medication in accordance with policy and individual health plans
n
- Accurately complete progress notes and documentation
n
- Build positive relationships with participants, families, carers, and stakeholders
n
- Ensure participant safety, well-being, and safeguarding requirements are met
n
- Promote participant rights, dignity, and self-determination
n

- You have flexible availability across 7 days between 8am - 6pm.

- You are willing to obtain a First Aid/CPR Certificate, NDIS Worker Screening Check, and Working with Children Check.

- You have a valid driver's license and an insured car to support people on their adventures.
