Job responsibilities:

Educate patients and caregivers on exercises, safety strategies, and home programs as appropriate. Communicate functional changes and barriers to the care team to support care planning. Collaborate with the care team to coordinate therapy plans and discharge recommendations. Work 40 hours per week with 8-hour shifts.
