Community Worker
Austa Care is a multicultural aged care and community services provider supporting elderly clients and culturally diverse communities across Australia. Due to continued growth in community- based aged care and well-being services, we are seeking an experienced Community Worker to support clients' social participation, independence, wellbeing, and connection with community resources.

Community Worker/ Community Support Worker
Full-time position
Subclass 482 sponsorship may be available for suitable candidates
Key Responsibilities
- Assess clients' social, emotional and community support needs through regular communication and observation
- Develop and assist with individual community participation, lifestyle and wellbeing support plans
- Organise and accompany clients to community activities, social outings, cultural events, appointments and interest-based programs
- Encourage elderly clients to maintain independence, confidence, routine, social connection and community involvement Provide emotional support, companionship and practical guidance to clients who may feel isolated or vulnerable
- Liaise with families, care coordinators, community organisations and service providers to ensure clients receive appropriate support
- Identify suitable local services, community resources, welfare programs and social support options for clients
- Monitor clients' progress, participation and wellbeing, and report concerns or changes to relevant supervisors
- Maintain accurate client notes,
activity records and basic progress reports in line with organisational procedures
- Promote culturally sensitive support for clients from diverse language and cultural backgrounds
- Assist with group-based lifestyle, wellbeing and community engagement programs where required
- Support clients to access information, build confidence and participate more actively in their local community
Important Role Clarification
This is a Community Worker role, not a primarily personal care role.
The main duties are community participation, social support, well-being support, client coordination, family liaison and service connection.
The role is not mainly focused on:
showering, toileting or heavy personal care medication administration or nursing duties manual handling as the primary function routine domestic cleaning as the core duty
Candidate Requirements
- Relevant qualification in Community Services, Aged Care, Disability, Social Work, Welfare, or a related field
- Previous experience in community services, aged care lifestyle support, social support, welfare support, or disability/community programs
- Robust communication and interpersonal skills
- Ability to work with elderly clients, families and multicultural communities
- Good record-keeping and reporting skills
- Compassionate, patient and culturally respectful approach
- Driver Licence and First Aid Certificate preferred
- Bilingual or multilingual ability will be highly regarded
