Role Description The NDIS Support Coordinator is a full-time, on-site role based in Sydney, NSW. In this position, the coordinator works closely with NDIS participants to understand their goals, explore available supports, and develop individualized plans that align with their needs and preferences. Day-to-day tasks include coordinating services, managing cases, liaising with service providers, and ensuring supports are implemented effectively and reviewed regularly.
The coordinator also helps participants build capacity to navigate the NDIS, monitors progress against goals, and responds to changes in circumstances or support needs.
The role involves maintaining accurate documentation, advocating for participants, and collaborating with internal and external stakeholders to achieve positive outcomes.
Qualifications
- Candidates should possess strong Interpersonal Skills and Communication abilities to build trust, collaborate with diverse stakeholders, and advocate effectively for participants.
- Candidates should possess Case Management skills to coordinate supports, monitor progress, and manage complex situations within NDIS frameworks.
- Candidates should possess Customer Service skills to respond promptly, professionally, and respectfully to participant inquiries and needs.
- Candidates should possess Technical Support skills to navigate NDIS portals, digital case management tools, and basic office software efficiently.
- Relevant experience in disability services, social work, community services, or a related field is highly beneficial.
- A qualification in social work, psychology, community services, or similar discipline, or equivalent experience in NDIS support coordination, is preferred.
- Knowledge of the NDIS, person-centered practice, and trauma-informed approaches is advantageous.
- Ability to work on-site in Sydney, NSW, with sound time management, organization, and documentation skills.
