Role Overview:

The Manager - Support at Home is responsible for leading and supporting the effective delivery of care coordination and management across the Support at Home program ensuring high standards of quality, consistency, and person-centred care. This role provides both operational and workforce leadership, combining supervision of the Intake Services, Care Partners (clinical and non-clinical) & Service Coordinators.

Key Responsibilities:

The Manager - Support at Home maintains the following primary responsibilities:

- Lead, manage and provide supervision to the Intake Officer, Care Partners and Service Coordinators within the CHSP & Support at Home program.
- Provide oversight of the assessment and service coordination within the Home and Community Care Program (HACC) ensuring that targets can be met
- Ensure the effective and efficient management of staff rosters.
- Drive and support recruitment processes across Support at Home, CHSP & HACC as required
- Drive service improvement initiatives to enhance the quality and consistency of care delivery.
- Foster a positive and collaborative team culture that embraces emotional intelligence
- Ensure high-quality, person-centred care is consistently delivered to Support at Home CHSP & HACC participants.
- Support staff in promoting client independence and well-being through tailored care planning and service coordination.
- Maintain strong relationships with clients, staff, and stakeholders to support person-directed care.
- Oversee the implementation of care plans in collaboration with the Support at Home, Intake CHSP, and HACC programs.
- Utilise data to monitor service performance, identify trends, and drive continuous improvement.
- Ensure compliance with regulatory and accreditation standards.
- Lead and support internal quality assurance activities and accreditation processes.
- Promote continuous improvement practices within the team.
- Ensure staff have up-to-date training, knowledge, and skills relevant to their roles.




- Strong understanding of KPI management and performance.
- Model skilled communication and a commitment to ongoing learning and development.
- This position is an identified Key Personnel position requiring specific compliance with Alpine Health’s Key Personnel Policy and Procedure.

Essential:

- Tertiary qualification in Allied Health or Clinical
- Training, experience and leadership in health and community services
- Experience in leadership, the supervision of staff and management of teams
