About The Role
The Senior Occupational Therapist works as part of the Geriatric Outreach Service (GOS), alongside another Occupational Therapist, Nurse Practitioner and Allied Health Assistant. You will provide specialised Occupational Therapy services to patients with dementia, cognitive impairment or diminished capacity who are preparing to transition from acute hospital settings into residential aged care.
Working collaboratively with acute services, Residential Aged Care Facilities, patients, families and carers, you will undertake assessment, care planning and intervention to support successful transitions and prevent unnecessary hospital transfers. You will also contribute to education, training and support to improve the overall care of patients and residents living with dementia.
The role is based in Mount Gambier, with travel across the Limestone Coast required as part of the position.
About You
You are an experienced Occupational Therapist with advanced clinical skills and demonstrated experience supporting older people with complex needs, particularly cognitive impairment, dementia and changed behaviours. You bring strong clinical leadership, communication and organisational skills and can work collaboratively across multidisciplinary teams, acute services and residential aged care settings.
To succeed in the Senior Occupational Therapist – Geriatric Outreach Service role, you will:
· Hold a degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia.
· Demonstrate experience in the assessment and provision of high-quality Occupational Therapy services to clients with cognitive impairment, dementia and/or changed behaviours.
· Demonstrate the ability to manage complex cases and provide specialised Occupational Therapy assessment, intervention and clinical advice.
· Have high-level interpersonal and communication skills,
including negotiation, consultation and the ability to establish effective relationships with patients, families, carers and key stakeholders.
· Demonstrate clinical management and leadership experience, including service development, quality improvement and the supervision of students, Allied Health Assistants and junior staff.
About the Geriatric Outreach Service
The Geriatric Outreach Service works across acute hospital and Residential Aged Care Facility settings within the Limestone Coast Local Health Network, with a focus on preventing unnecessary hospital admissions and transfers.
The team reviews and supports patients with dementia or diminished capacity who are preparing to transition into residential aged care. Working collaboratively with acute services and Residential Aged Care Facilities, the team provides assessment, care planning, education, training and support to improve the overall care and wellbeing of patients and residents.

Conditions of Employment
· A current Nationally Coordinated Criminal History Check (NCCHC) for employment involving unsupervised contact with vulnerable groups is required for this position.
· Immunisation Risk for this position is – Category A.
· SA Health Services are required to implement the Addressing vaccine preventable disease: Occupational assessment, screening and vaccination policy
