About us
Connected Well supports Primary Care health services in Canowindra. Our principal client is a popular rural GP who delivers pro-active, carefully-planned care to a chosen panel of patients. In 2027 we expect to grow to support additional visiting GPs and a recent nurse-led clinic.
The Position
There are three area’s the nursing staff are required to work in under this position for Connected Well:
1. Nurse Led Clinic - Triage & Work-Up: RN’s will be required to triage, perform Primary & Secondary assessments, and co-ordinate care navigation to manage minor presentations within their scope of practice using local resources. RN’s will then be required to present the GP with the complete clinical picture of the patient to assist the GP in their job of diagnosing & overseeing the management of patient medical conditions.
2. GP Clinic - Supporting a Dr's patients with Chronic disease management: RN’s will be required to develop an ongoing clinical relationship with the Dr’s chronic disease patients to supervise and sub co-ordinate chronic disease management/patient journeys through the health care system.
3. GP Clinic - the usual community-focused Procedures: immunisation, wound dressings, assisting with minor procedures, etc.
We are looking for an engaged, committed and enthusiastic Registered Nurse who is interested in operating across all three work areas of Connected Well and has a clear understanding of their scope of practice.
Prior primary health care nursing experience is not essential, but interest is required. We will support all nursing staff within this role to grow and expand their scope of practice with training, clear supervision and regular contact with GPs.
The Situation
This position is currently casual; however,
it has the potential to become permanent part-time for the right candidate.
We offer:
- at least 20 hours work per fortnight;
- at least 2 weeks' advance notice of shifts;
- no night shift;
- a supportive team environment for when life happens.
The ideal candidate would be available for shifts between the hours of 8.30am – 5.30pm and ready to start mid to late October 2026
Essentials
- Minimum 2 years' nursing experience as a Registered Nurse
- Current AHPRA registration
- A current NSW Drivers Licence.
- Interest in Primary Care / General Practice
- Immunisation Certificate or good knowledge of the NSW vaccination schedule
- Current CPR certification
- Excellent communication and teamwork skills
- Ability to work independently and prioritise tasks
- Capable computer skills
- Australian permanent residency required
Key Responsibilities
Routine community health:
- Triage and patient assessments
- Childhood & adult immunisations
- Wound care and chronic wound management
- Ordering and organising diagnostic tests & investigations
Assisting GPs with procedures including:
- Skin biopsies
- Wound closure
- Medication administration
- Ear irrigation
- Other minor procedures
Chronic Disease Management:
- 75+ Health Assessments (and other HA's)
- chronic disease care plan development
- chronic disease patient case management
- Patient recalls, reminders, and follow-up
Other:
- First-line emergency response and use of emergency equipment
- Supporting the Cold Chain 'Strive for 5' processes
- Infection control and sterilisation
- Supporting practice accreditation requirements
