Key responsibilities may include:
Providing person-centred support based on each participant’s individual needs, preferences and goals
Assistance with personal care and daily living activities
Supporting community access, appointments and social participation
Supporting participants to build and maintain independence
Medication assistance in accordance with training and participant care plans
Supporting participants with complex and high-intensity support needs, where appropriately trained
Following individual support plans, health care plans and risk management strategies
Maintaining accurate progress notes and required documentation
Communicating effectively with participants, families and other members of the support team
Providing High intensity care

Pay: $40.00 – $47.00 per hour

Expected hours: 10.0 – 30.0 per week
