NDIS Disability Support Worker – Sydney

Hometown Nursing Care is looking for compassionate, reliable and motivated Disability Support Workers to join our growing NDIS team across Sydney.

You will provide person-centred support to NDIS participants, helping them live independently, participate in their community and achieve their individual goals.

Key Responsibilities
Provide personal care and assistance with daily living
Support participants with community access and social activities
Assist with appointments, shopping and everyday tasks
Encourage independence, choice and confidence
Follow individual NDIS support plans
Maintain accurate progress notes and documentation
Work respectfully with participants, families and other professionals
Follow Hometown Nursing Care policies and NDIS requirements

What We’re Looking For
Certificate III/IV in Individual Support, Disability or a related qualification preferred
Previous disability/NDIS experience highly regarded
NDIS Worker Screening Check




First Aid & CPR
Working With Children Check where required
Australian working rights
Current driver licence and reliable vehicle preferred
Excellent communication skills
Caring, respectful and dependable attitude
