IUIH is seeking a Clinical Care Coordinator – MATSICHS Social Health to provide advanced clinical assessment, triage, and coordination of supports for Aboriginal and Torres Strait Islander clients with complex social and mental health needs.

The role requires high-level clinical judgement, collaboration with Social Health practitioners, and leadership in intake, triage, and care planning to ensure protected outcomes for clients within Caboolture, Morayfield, Strathpine area and beyond.
