Geraldton Regional Aboriginal Medical Service (GRAMS) is seeking a passionate
Aboriginal Health Practitioner (AHP)
to join our team.
Working within a multidisciplinary team, you will provide culturally safe primary health care, health education and ongoing support to Aboriginal clients both in the clinic and the community.
About the Role
As an Aboriginal Health Practitioner, you will:
Deliver culturally appropriate primary health care services.
Conduct health assessments and provide health education.
Support chronic disease management and preventative health programs.
Work collaboratively with GPs, nurses and visiting specialists.
Promote health and wellbeing through community engagement.
Participate in continuous quality improvement and health promotion initiatives.
Current registration as an Aboriginal Health Practitioner with AHPRA.
Demonstrated understanding of Aboriginal health and cultural issues.
Strong communication and interpersonal skills.
Ability to work independently and within a multidisciplinary team.
Current unrestricted driver's licence.
National Police Clearance and Working with Children Check (or ability to obtain).
