Job Description
St John of God Murdoch Hospital is seeking a compassionate palliative care coordinator to plan, deliver and evaluate meaningful programs for patients, carers and families within a multidisciplinary setting.
You will work with clinicians and external providers to ensure seamless care across the continuum, fostering connections with community services and promoting holistic well-being for patients and families.
The role is fixed-term, 12 months, part-time (20 hours per week) with adaptable options.
