Care Partner | Support at Home
Are you an experienced Case Manager or Care Coordinator with a passion for helping older people live independently at home?
Our amazing client is seeking a compassionate and proactive Care Partner to join their growing team. This is an exciting opportunity to play a pivotal role in the delivery of Australia's new Support at Home Program, working closely with clients to achieve meaningful outcomes through person-centred care planning, coordination, and ongoing support.
About the Role
As a Care Partner, you will act as the primary relationship manager and designated care manager for a portfolio of older Australians receiving Support at Home services.
You will partner with clients, families, support networks, allied health professionals, and service providers to develop and manage tailored care plans that promote independence, wellness, reablement, and quality of life.
This role is ideal for someone who enjoys building strong client relationships while managing all aspects of the case management cycle, including:
- Comprehensive assessments and care planning
- Care coordination and service delivery oversight
- Ongoing client reviews and monitoring
- Budget and package management
- Risk identification and escalation
- Stakeholder engagement and case conferencing
- My Aged Care and Support at Home compliance requirements
What You'll Be Doing
Managing a caseload of older clients receiving Support at Home services
Developing and reviewing personalised care plans aligned with client goals and needs
Coordinating services across care workers, nursing, allied health, and external providers
Conducting ongoing case management activities and regular client reviews
Monitoring client outcomes, risks, service quality, and changing care requirements
Supporting clients through reablement, restorative care and end-of-life care pathways
Managing individual budgets and ensuring appropriate package utilisation
Assisting clients to navigate My Aged Care processes, reassessments, and support plan reviews
Building trusted relationships with clients, families, carers, and stakeholders
Maintaining accurate documentation and ensuring compliance with Aged Care Quality Standards
About You
To be successful, you will bring:
- Demonstrated experience in Case Management, Care Coordination, Care Partner, Support Coordinator, Care Advisor, or similar roles
- Previous experience within Aged Care, Home Care Packages, Community Care, Disability Services or Health Services
- Strong understanding of the Support at Home Program, Home Care Package environment, or community aged care services
- Experience managing client care plans, reviews, service coordination, and stakeholder engagement
- Sound financial literacy, including budget monitoring and package management
- Excellent communication, relationship-building and problem-solving skills
- Strong technology skills and experience using CRM or care management systems
- Relevant qualification in: Community Services, Aged Care, Case Management, Nursing,Allied Health, or equivalent
Highly Regarded
Experience with Support at Home transition activities
Familiarity with My Aged Care systems and processes
Experience supporting clients with dementia, chronic health conditions, or palliative care needs
Knowledge of reablement, restorative care and wellness approaches

Requirements
Current Police Check or NDIS Worker Screening Check
Current Driver's Licence and reliable vehicle
Current First Aid and CPR Certificate (or willingness to obtain)
Evidence of required vaccinations
