BDAC in the Bendigo region is seeking a Chronic Care Coordinator to deliver culturally protected, short-term care coordination for Aboriginal and Torres Strait Islander community members living with cardiovascular disease, respiratory disease and/or diabetes who are at risk of hospitalisation or fragmented care.
Working within BDAC's Chronic Disease Management Team, the role focuses on stabilising health needs, strengthening self-management, and connecting clients with ongoing supports.
