JOB DESCRIPTION

- We are seeking an AN-ACC Coordinator to support multiple residential aged care homes, ensuring assessments are accurate, funding opportunities are maximised, and compliance standards are consistently achieved.
n
- Based in Rivervale
n
- Lead AN-ACC processes across multiple residential aged care homes
n
- Review and support assessment accuracy and documentation quality
n
- Identify opportunities to optimise AN-ACC funding outcomes
n
- Support compliance with aged care funding requirements
n
- Partner with site leadership teams and key stakeholders
n
- Undertake regional travel as required
n

n

About the Candidate

n

n
- 3-5 years' experience with AN-ACC funding processes
n




- Strong understanding of the AN-ACC funding model
n
- Experience within residential aged care
n
- Excellent communication and stakeholder engagement skills
n
- Strong computer literacy and attention to detail
n
- Current driver's licence and willingness to travel
n
- Clinical Care Coordinators
n
- Quality & Compliance Coordinators
n
- Quality Managers
n
- Residential Aged Care Clinicians
n
- Registered Nurses with robust AN-ACC experience
