Albury Wodonga Health in NSW seeks a Palliative Care Social Worker to join the Community Palliative Care Team. You will undertake psychosocial assessments, provide counselling, advocacy and coordinate care for patients, carers and families in community and outpatient settings.

This permanent part time role (32 hours per week) offers professional development, supportive supervision and a chance to contribute to high-quality palliative care services.
