As a Clinical Care Coordinator, you'll play a vital role in supporting people throughout their recovery journey by providing clinical assessments, care planning, counselling, case management, and coordination of services to ensure positive, sustainable outcomes.

About the Role

n

Working as part of a multidisciplinary team, you will:

n

n
- Conduct comprehensive mental health assessments and risk assessments
n
- Develop personalised treatment and recovery plans
n
- Deliver evidence-based therapeutic and counselling interventions
n
- Manage an ongoing clinical caseload
n
- Coordinate care with families, GPs, hospitals, allied health providers, and community services
n
- Support clients transitioning between community and tertiary mental health services
n
- Participate in case reviews, clinical supervision, and quality improvement activities
n
- Advocate for client wellbeing through a person-centred and recovery-oriented approach
n

What We're Looking For

n

We are seeking clinicians who have:

n

n
- Experience working with complex mental health presentations
n
- Strong counselling and therapeutic intervention skills
n
- A recovery-oriented and trauma-informed practice approach
n
- Exceptional communication and stakeholder engagement abilities
n
- Experience working collaboratively within multidisciplinary teams
n
- Understanding of culturally safe practices, particularly when working with First Nations communities
n

Essential Requirements

n

n
- Relevant tertiary qualification in:
n
- Psychology
n
- Social Work
n
- Mental Health Nursing
n
- Occupational Therapy
n
- Or a related discipline
n
- Current AHPRA registration (where applicable) or eligibility for AASW membership
n
- Current Driver's Licence
n
- National Police Check
n
- Working with Children Check (or ability to obtain)
n
- Full Australian working rights
n
