Clinician - Tokoko Outreach

Flexible | Remote, Hybrid or Office-Based | 30.4 hours per week | Fixed-Term to 30 June 2027

SCHADS Level 5 | $105,570 - $110,351 per annum (pro rata) + Superannuation + Salary Packaging Benefits

We are seeking a qualified and passionate Clinician to join our newly funded Tokoko Integrated Recovery Pathways Program.

This innovative role will help strengthen recovery outcomes for people accessing alcohol and other drug treatment and recovery services across the Wide Bay region. Through ConnectWell, Lives Lived Well's digital wellbeing platform, you will help people stay connected to support throughout their recovery journey and improve access to services beyond traditional service settings.

About the Role

As the Clinician, you will play a key role in strengthening continuity of care for people engaging with alcohol and other drug treatment and recovery services.

Using ConnectWell, you will provide flexible, recovery-focused support that helps individuals remain connected throughout their treatment and recovery journey. This includes supporting engagement before admission, maintaining connection during treatment, providing follow-up support after discharge and assisting individuals to re-engage with services when additional support is needed.

Working collaboratively with the broader Tokoko Integrated Recovery Pathways team, you will help ensure people have access to timely support, information and service connections regardless of where they are in their recovery journey.

Key Responsibilities

- Deliver recovery-focused support and engagement through ConnectWell's digital platform.
- Conduct regular recovery check-ins with individuals preparing for, participating in or transitioning from residential rehabilitation.




- Support ongoing client engagement and continuity of care throughout treatment and recovery.
- Provide follow-up support for individuals following discharge from residential rehabilitation.
- Assist individuals to access appropriate alcohol and other drug, health, wellbeing and community support services.
- Support re-engagement for people who may be at risk of disengaging from treatment or recovery supports.
- Work collaboratively with the wider Tokoko Integrated Recovery Pathways team to strengthen coordinated care and recovery outcomes.
- Promote recovery-oriented, strengths-based and trauma-informed practice.
- Contribute to service development, client engagement initiatives and continuous improvement activities.
- Maintain accurate documentation, reporting and client records.

This role can be undertaken remotely, in a hybrid arrangement or from a Lives Lived Well office location, subject to operational requirements. What We're Looking For To succeed in this role, you will bring:

- Qualifications in Social Work, Psychology, Counselling, Occupational Therapy, Nursing, Human Services or a related discipline, and/or relevant experience.
- Experience working within the Alcohol and Other Drug, Mental Health or community services sector.
- Experience supporting engagement, recovery, care coordination, wellbeing or continuity-of-care initiatives.
- Strong understanding of recovery-oriented, trauma-informed and person-centred practice.
- Knowledge of alcohol and other drug treatment, recovery and harm minimisation principles.
- Ability to engage effectively with people experiencing complex needs.
- Strong communication, relationship-building and engagement skills.
- Confidence using digital platforms and technology to engage and support clients.
- Ability to work independently while contributing positively within a multidisciplinary team.
- Current Australian Driver Licence.

Experience in telehealth,



digital service delivery or online client engagement will be highly regarded.
