REGISTERED NURSES (RN) & PERSONAL CARE WORKERS (PCW)

Location: Horsham, Victoria

Registered Nurse (RN)

In this role, you will deliver high-quality clinical nursing care to clients in their homes and within the community, supporting their health, independence, and overall wellbeing.

Your duties may include, but are not limited to:

- Delivering clinical nursing care in accordance with professional standards and individualised care plans

- Administering medications and monitoring client health status

- Providing wound care, continence care, catheter care, PEG feeding, and other nursing interventions as required

- Maintaining accurate, timely, and professional clinical documentation

- Collaborating with clients, families, and multidisciplinary teams to promote optimal outcomes

- Escalating clinical concerns appropriately

- Contributing to quality improvement and safe practice initiatives

Personal Care Worker (PCW)

In this role, you will play a vital part in supporting clients to live independently and with dignity in their own homes. You will provide personal care, domestic assistance, and companionship, while ensuring a safe, respectful, and person-centred approach to care.

Your duties may include, but are not limited to:

- Provide domestic assistance including cleaning, laundry, tidying, meal preparation, and maintaining a safe and comfortable home environment

- Delivering high-quality personal care, including showering, toileting, grooming, dressing, and assistance with meals

- Monitoring and observing changes in client health or behaviour,



and reporting concerns promptly to the care team

- Providing emotional support and meaningful companionship to promote client well-being

- Maintaining accurate and professional documentation, including progress notes and communication with team members

- During overnight shifts, conduct regular wellbeing checks, respond promptly to alarms, calls or distress, and provide reassurance

Mandatory Requirements

- Current APHRA Registration (For Registered Nurse)

- Current working rights in Australia

- Current Police Check (within last 12 months)

- Employee Working with Children Check

- NDIS Worker Screening Check

- Current Flu Vaccination (2026)

- CPR and First Aid Certificates

- COVID-19 Vaccination

- Current Victorian Driver’s License
