Comfort N Care Support QLD is looking for a Service Delivery Coordinator to join our growing team in a newly created, permanent part time position.

This is a hands-on role for someone who knows the disability sector, enjoys taking ownership of their work and can bring a calm, organised approach to a busy service environment.

About the role

Working closely with our Directors and rostering team, you’ll coordinate the participant side of our services and help ensure supports are safe, person-centred, consistent and delivered by appropriately trained staff.

You’ll be a key point of contact for participants, families, support workers and external stakeholders, while helping maintain the quality systems and documentation that keep our services running smoothly.

What you’ll be doing

- Coordinating participant onboarding, service delivery, reviews and ongoing satisfaction.

- Maintaining service agreements, support plans, risk assessments and participant documentation.

- Supporting the implementation of Positive Behaviour Support Plans (PBSPs) and ensuring staff understand participant-specific requirements.

- Identifying training needs and organising staff training, including manual handling, medication assistance, seizure management and other specialist supports where applicable.

- Monitoring staff competencies and ensuring required training and documentation remain current.

- Providing day-to-day guidance, supervision and support to disability support workers.





- Working with rostering to ensure participants are matched with suitable and appropriately trained staff.

- Following up participant feedback, complaints, incidents and service delivery concerns.

- Liaising with families, Support Coordinators, allied health professionals and other stakeholders.

- Assisting with quality improvement, compliance and the development of practical service delivery processes.

- Escalating complex matters and significant operational decisions to the Directors.

What we’re looking for

You’ll ideally have:

- Demonstrated experience in disability service delivery, team leadership or a senior coordination role.

- Strong knowledge of the NDIS and person-centred support.

- Practical experience working with Positive Behaviour Support Plans.

- Experience supporting or supervising disability support workers.

- Confidence coordinating training and participant-specific competency requirements.

- Strong organisational skills and attention to detail.

- Experience with support plans, risk assessments, incidents and participant documentation.

- Excellent communication skills and the ability to build positive relationships with participants, families and staff.

- A proactive, solutions-focused approach and the ability to manage competing priorities.

- A genuine commitment to participant choice, dignity, safety and quality of life.

Experience with SIL, complex support needs and NDIS quality systems will be highly regarded.

You’re approachable, skilled and comfortable having difficult conversations when needed. You understand that good service delivery is about both people and processes—and that neither works particularly well without the other.

Location: Toowoomba, Queensland

Employment Type: Permanent Part-Time – 3 days per week

Remuneration: $46.70 per hour plus superannuation, subject to the applicable SCHADS Award classification and pay point.
