Colac Area Health is seeking an assessment clinician to work in the Aged Care Assessment Program across the VIC 3 region. You will conduct comprehensive assessments across physical, medical, psychological, cultural and social dimensions and refer clients to services including residential care, respite and community packages.
You will collaborate with clients to determine needs and goals, using wellness and reablement to promote independence.
