healthAbility is seeking a Reconnect Counsellor & Case Manager to support young people aged ***** at risk of homelessness in Banyule or Nillumbik.
The role combines case management and counselling within a multidisciplinary team and includes substantial outreach.
The position is 0.6 FTE until 30 June ****, with hybrid working arrangements and a focus on community networking to improve wellbeing and life-skills.
