The Role As a Clinical Care Coordinator, you will:

Manage a clinical caseload and provide direct mental health support
n
Conduct comprehensive assessments, case formulations and care planning
n
Deliver evidence-based therapeutic interventions
n
Undertake risk assessments and develop risk management plans
n
Coordinate care across clients, families, healthcare providers and community services
n
Facilitate referrals and support transitions between services
n
Develop strong community and stakeholder relationships
n
Contribute to clinical governance, reporting and quality improvement initiatives
n
n
About You
n
You are a compassionate and experienced mental health professional who is passionate about recovery-focused care.

You'll bring:

Experience supporting people with complex mental health presentations
n
Strong understanding of community mental health services
n
Trauma-informed and recovery-oriented practice experience
n
Excellent communication and relationship-building skills
n
Ability to work effectively within multidisciplinary teams
n
A person-centred, non-judgemental and culturally responsive approach
n
n
Essential Requirements
n
n
Relevant tertiary qualification in:n
n
Psychology
n
Social Work
n
Mental Health Nursing
n
Occupational Therapy
n
Or a related mental health discipline
n
n
Current skilled registration or eligibility relevant to your discipline
n
Current Australian driver's licence
n
National Police Check
n
Working With Children Check (or applicable exemption)
n
