UnitingCare Queensland's BlueCare division is hiring a Clinical Care Partner based in Caboolture and surrounding areas to coordinate complex clinical and care needs, ensuring person-centred services across the community. Develop and review care plans with clients and the multidisciplinary team, manage budgets and navigate funding streams, liaising with clinical staff to monitor outcomes.
