We're seeking a committed Community Nurse to join our team and help improve the lives of older adults living independently in the community.In this rewarding role, you'll draw on primary health care principles to deliver holistic, person-centred care to older adults experiencing chronic conditions and complex needs.
Working as part of a multidisciplinary team, you'll provide early intervention, health education, and recovery-focused support within clients' homes and community settings.This is a meaningful opportunity to make a real difference in the lives of older people—helping them stay well, stay safe, and stay connected.About The RoleProviding direct clinical nursing care to our clients in their homes in line with best practice nursing proceduresThe duties will be varied and will include clinical assessments, dressings, medication assessment and catheter changingDevelop a holistic nursing care plan in collaboration with client, carer, Care Advisor and Allied health where relevantProvide safe, quality nursing practice to clients with specific needs and/or complex needs eg.
Wound care, palliative care,
education and supportLiaise with all parties involved the care of the client to maintain holistic health care and wellbeingEnsure all documentation is accurate and completed in a professional and timely mannerWorking in a safe manner to protect yourself, your colleagues and clients closely following all policies and proceduresTo be successful in this role you'll needA kind heart and genuine passion for providing high quality care to the elderlyCurrent AHPRA registration following completion of a Bachelor of Nursing or similarMinimum 3 years post graduate nursing experienceExperience in clinical assessments – PAS & RUDASStrong communication skills and a positive attitudeSound computer skills especially Microsoft Office products and ability to pick up new systemsDrivers license and a reliable vehicleWillingness to apply for a Police Check and NDIS clearanceWhat We Can Offer YouCompetitive hourly pay rate and kms reimbursed ($1.11 per km)No Shift work!
