About the role
This role is an exciting opportunity to work with the Respiratory Coordinated Care and Chronic Care Team to enhance your skills and knowledge while improving the health outcomes of people with respiratory conditions. Comprehensive orientation, along with ongoing mentoring, training, and support, will be provided.

You will work with a diverse range of clients and carers across the lifespan within a multidisciplinary team in outpatient, community, and inpatient settings.

The position works collaboratively within a small multidisciplinary team and is based within the Primary and Community Health Team. Work hours are Monday to Friday, from 8:00/8:30 am to 4:30/5:00 pm.
