About the Opportunity

Note: This is a short term (12 month) part-time position (3 days per week).

As the YHOPP Case Manager, you will provide intensive, trauma-informed and youth-focused case management support to young people aged 10 to 25 years who are experiencing or at risk of homelessness.

Working within a strengths-based and culturally safe framework, you will support young people to access and sustain safe accommodation, strengthen family and community connections, develop independent living skills, and achieve positive long-term outcomes.

Through outreach, advocacy, assessment, and collaborative case planning, you will help young people navigate complex challenges and connect with the services and supports they need.

To be successful in this role, you will bring:

- Qualifications in Human Services, Social Sciences or a related discipline
- Experience supporting vulnerable or high-risk young people and families.
- Knowledge of the youth homelessness,



out-of-home care, youth justice and health service systems.
- Demonstrated experience applying trauma-informed, strengths-based case management practices.
- Excellent engagement skills and the ability to work effectively with young people experiencing complex need.
- A commitment to culturally safe and inclusive practice, particularly when working with Aboriginal and Torres Strait Islander peoples and culturally diverse communities.
