As a Care Coordinator with Care1, you will be responsible for working closely with our dedicated team to coordinate services for our participants in Greater Sydney, Queensland, Canberra, and regional areas within NSW.

About you
We're looking for an excellent communicator that is confident, passionate and self-motivated. We are looking for an individual that would love to work in a high performing culture; an cutting-edge thinker with the ability to multitask and manage competing priorities.
You will:
- be able to work autonomously to manage your own workload,
- have a positive attitude, and the ability to think creatively to solve problems and promote solutions,
- have customer service experience in healthcare, disability, aged care or related industry is beneficial but not essential,
- feel comfortable using various systems including rostering systems and CRM and MS Office applications,
- have the ability to establish and maintain relationships with a wide variety of key stakeholders,
- have outstanding attention to detail along with excellent verbal and written communication skills,
- be able to attend client / stakeholder meetings through out NSW and interstate periodically.
About the Role
Key responsibilities include:
- rostering staff to match client needs,
- building and maintaining positive relationships with staff, clients and their families & participating in client case meetings,
- training staff on internal practices and coordinating client specific training as needed,
- promoting the services delivered by Care1 in a positive manner,
- ensure care plans are current and are communicated with the support team & ensure familiarity with client changes needs and provide direction and support to the care team as appropriate,
- you will play an active role in recruiting staff suitable to meet the needs of our clients,
- using various systems including rostering systems and CRM and MS Office applications,
- responding to incident reports, and ensure your documentation is complete will be valuable in this role,
- part of your ongoing employment duties will involve being rostered for ‘after hours co-ordination’ for approximately one evening a week and 1 weekend a month. This is approximate based on the available Care Coordinators. You will receive an allowance for any time you are on call as per the on call roster and some post on-call time off after weekend on-call (currently 3 hours).

- capacity to take on-call responsibility after hours on a rotating roster (this is currently one evening a week and one weekend a month)
