About the role

This position exists to build Pursue Ability's Support Coordination business. The Support Coordinator inherits an existing caseload and is responsible for growing it by bringing current participants to Pursue Ability, holding them, and delivering enough coordinated support to sustain the service line commercially. The position is part time, with hours set by reference to the caseload carried. The billable target for a fully built caseload is 35 claimable support coordination hours per week.

Key responsibilities

- Generate and convert Support Coordination referrals into active participants
- Build and hold working relationships with referral sources including Local Area Coordinators, NDIA planners, plan managers, hospital and health discharge teams, allied health practitioners, general practitioners, community and advocacy organisations, and existing participants and their families
- Maintain a live referral pipeline, recording every enquiry, its source, its status and its outcome in the client management system
- Respond to every new referral within one business day
- Identify participants within Pursue Ability's existing service lines who hold unspent or unallocated Support Coordination funding and convert those to active coordination engagements where appropriate
- Deliver Support Connection, Coordination of Supports and, where qualified and assigned, Specialist Support Coordination in accordance with the participant's plan and the NDIS Pricing Arrangements and Price Limits
- Achieve 35 claimable support coordination hours per week at full caseload,



measured on a four-week rolling average
- Record service delivery contemporaneously, with case notes entered within 24 hours of service delivery
- Implement each participant's plan by connecting participants to supports, negotiating service agreements, and building the participant's capacity to direct their own supports
- Prepare and lodge NDIA reports on time, evidenced and written to a standard that withstands review

About you

- Demonstrated experience in NDIS Support Coordination, or in a case management, community services or allied health role with direct transferable skills
- Working knowledge of the NDIS, the NDIS Practice Standards, the NDIS Code of Conduct and the Pricing Arrangements and Price Limits
- Demonstrated ability to generate referrals and build a caseload, or clear evidence of the relationship-building and business development capability required to do so
- Demonstrated ability to work to a billable target and to manage own time without close supervision
- Sound written communication, with ability to lodge reports with the NDIA that are evidenced, defensible and on time
- Sound judgement in risk, safeguarding and escalation
- Competent use of client management systems and standard office software
- Current NSW driver licence and a comprehensively insured, roadworthy and registered vehicle
- NDIS Worker Screening Check clearance
- Current First Aid and CPR certification
