Silverchain is seeking experienced Home Care Workers to join our Transitional Aged Care Program (TACP) in the Manly area. This role provides in-home rehabilitation-focused support for older clients returning home from hospital.

You will assist with personal care, mobility activities, meal prep, transport to appointments, and social engagement under supervision. Training in hoist use and compression stockings is provided; roster is Mon-Fri 7am–5pm with 2–3 days per week required.
