Uniting Vic.Tas in Blackburn, VIC is seeking a Client Assessment and Support Planner to work with younger people with disability or health conditions and their carers, helping them access supports to live as independently as possible within HACC PYP.

You will conduct holistic, strengths-based assessments, often in clients’ homes, develop practical plans, research services, coordinate referrals and connect clients with community, health and government-funded supports that best meet needs.
