We are currently recruiting on behalf of a well-established community-focused in-home aged care provider with over 40 years' experience, based in Melbourne's north-eastern suburbs, dedicated to supporting older Australians from multicultural communities to live independently, with dignity and choice.

We are seeking an experienced Care Partner who is passionate about aged care, client outcomes and delivering personalised support.

The Role

- Manage a caseload and provide end-to-end care coordination
- Develop and review personalised care plans, goals, OHS and emergency plans.
- Coordinate services within client needs and available budgets
- Build solid relationships with clients, families and service providers
- Maintain accurate client records and case notes




- Proactively respond to changing client needs and resolve issues

About You

- Relevant degree in Aged Care, Social Studies, Welfare, Case Management or a related field
- Previous experience as a Care Partner within Support at Home (SAH) - essential
- Strong understanding of person-centred aged care and care planning
- Excellent communication, organisation and problem-solving skills
- Confident using computer and client management systems
- Ability to work autonomously and as part of a multidisciplinary team
- Macedonian or Greek speaking - highly desirable
- Genuine passion for supporting older people and multicultural communities
