Company Description Guided Connect is a Victorian-based NDIS provider dedicated to supporting people to live meaningful, connected, and self-directed lives. The organisation specialises in working with individuals who have complex support needs, including psychosocial disability, mental health challenges, forensic and justice-related backgrounds, neurodiversity, and behaviours of concern, as well as those requiring Supported Independent Living (SIL) and community-based supports. Guided Connect uses a person-centred, trauma-informed approach, building genuine relationships and collaborating with participants, families, support coordinators, allied health professionals, and other stakeholders. Services range from disability support, SIL, and community access to complex behaviour supports, psychosocial recovery, specialist disability support workers, and support coordination. The team invests in skilled, compassionate staff and continuously improves practices to deliver high-quality supports that promote independence, wellbeing, safety, inclusion, and choice.

Role Description The Disability Support Worker (Complex Needs) role is a hands-on position providing direct support to NDIS participants with complex needs in Ballarat, VIC. This is an on-site role that involves supporting individuals in their homes and the community to build independence, achieve personal goals, and participate safely in everyday life. Day-to-day tasks include assisting with personal care, daily living activities, and community access, while implementing person-centred and trauma-informed support plans.



The role requires monitoring wellbeing, responding appropriately to behaviours of concern, maintaining accurate documentation, and following safeguarding and risk management procedures. The Disability Support Worker collaborates with participants, families, support coordinators, and allied health professionals to ensure consistent, respectful, and high-quality support.

Qualifications

- Experience working as a Disability Support Worker or in a similar support role, ideally with people who have complex needs or psychosocial disabilities.
- Strong interpersonal skills, including empathy, patience, and the ability to build trust and maintain professional boundaries.
- Sound understanding of safeguarding principles, risk management, and duty of care in disability and community settings.
- Explicit and respectful communication skills, including the ability to follow support plans, document shifts, and liaise with multiple stakeholders.
- Basic technical support skills relevant to the role, such as using electronic care systems, mobile apps for documentation, and assistive technologies.
- Certificate III or IV in Disability, Community Services, Mental Health, or a related field (or working towards qualification).
- Knowledge of NDIS practice standards, person-centred and trauma-informed approaches, and behaviour support frameworks.
- Current First Aid and CPR certification, NDIS Worker Screening Check, and valid driver’s licence.
- Ability to work shifts as required, including evenings, weekends, and sleepovers,
