St John of God Murdoch Hospital is seeking a Social Worker to join a multidisciplinary team with solid clinical social work practice. You will deliver services to patients, families and carers, supporting medical, nursing and allied health staff while promoting patient-centred care and health literacy.
You must have a degree in social work and be eligible for AASW membership, with skills in assessment, case management and discharge planning within hospital settings, including aged care.
