Your Role

As our Care Ambassador, you will support the delivery of high-quality community care from a client’s initial onboarding and throughout their ongoing care.

You will provide day-to-day leadership across client care by supporting caregivers to meet expectations, addressing issues early and helping the wider team maintain consistently high standards.

You will help ensure every client receives protected, consistent and personalised support. You will also work alongside our caregivers, making sure they understand each client’s needs, feel well prepared for their visits and receive timely guidance and support.

This role would suit a self-driven nurse with community nursing experience who enjoys building relationships with clients and families, supporting care teams, solving day-to-day problems and taking ownership of care delivery.

What Your Day-to-Day Looks Like





Client First Visits and Continuity of Care: Conduct first-visit introductions, support client onboarding, assist clients to review and complete service agreements, and review care plans and progress notes in AlayaCare. Help ensure each client’s care remains current, safe and personalised as their needs change.

Supporting Caregivers in the Field: Ensure caregivers understand each client’s needs, care priorities and visit expectations. Provide practical guidance, feedback and support so caregivers can deliver confident, consistent and compassionate care.

Hands-On Care and Shift Coverage: Provide direct care to clients and regularly step into scheduled visits when shifts require coverage. This is an important and expected part of the role, helping maintain continuity of care while giving you first-hand knowledge of our clients, caregivers and service standards.

Scheduling and Team Collaboration: Work closely with scheduling and the leadership team to respond to roster changes, cancellations, urgent care requirements and other issues affecting service delivery.

Care Reviews: Conduct supervisory visits, gather feedback from clients, families and caregivers, identify changes in client needs and escalate clinical or service concerns appropriately.

Documentation and Quality: Maintain accurate and timely documentation, monitor progress notes and contribute to quality and compliance processes to ensure care meets professional, regulatory and Nurse Next Door standards.
