About the Role

As a Care Partner, you will:
Manage and coordinate a caseload of Support at Home, NDIS, and private clients
Conduct client assessments, reviews, and care planning
Build meaningful relationships with clients, families, and healthcare professionals
Ensure clients receive services that meet their goals and changing needs
Monitor budgets, services, compliance, and client outcomes
Advocate for clients and provide exceptional customer service

About You

You’ll shine in this role if you’re organised, empathetic, and passionate about improving lives.

Must-haves:
Experienced in aged care, community care, case management, or care coordination




Background in community or aged care service coordination
Excellent communication and planning skills
Ability to work independently and as part of a team
Current driver’s licence
Able to obtain a Police Check & NDIS Worker Screening

Bonus points if you have:
Nursing qualifications (RN or EN)
