Role Description The Support Coordinator is a full-time, hybrid role based in Central Coast, NSW, with some flexibility to work from home. Day-to-day, the Support Coordinator will assist NDIS participants to understand their plans, identify appropriate services, and coordinate supports across multiple providers.

Responsibilities include developing support plans, managing service bookings, monitoring progress toward goals, and resolving issues or barriers to service delivery. The role involves regular communication with participants, families, and external stakeholders, maintaining accurate records, and ensuring services align with NDIS guidelines and organisational policies.

The Support

Coordinator will also provide guidance on available resources, advocate for participant needs,



and contribute to continuous improvement of support coordination processes.

Qualifications
Solid interpersonal skills and the ability to build respectful, cooperative relationships with participants, families, and providers.
Excellent communication skills, including clear written documentation and confident verbal communication with diverse stakeholders.
Experience with case management, including planning, coordinating services, and monitoring outcomes for individuals with complex needs.
Customer service capabilities, with a focus on responsiveness, empathy, and problem-solving in participant-facing situations.
Basic technical support skills and confidence using digital tools, client management systems, and online NDIS platforms.
Knowledge of the NDIS framework, disability sector, and person-centred practice.
Ability to work independently, manage competing priorities, and maintain accurate records in a hybrid work workplace.
Relevant qualification in community services, social work, disability, or a related field, or equivalent experience.
Valid driver’s licence and willingness to travel within the Central Coast and Lake Macquarie regions as required.
