Job Description
Western Health is seeking a Grade 2 Clinical Pharmacist – Transitions of Care to join the Pharmacy team.
You will focus on improving medication safety as patients move between hospital, home and other settings, working with patients, carers and multidisciplinary teams to ensure information is accurate and acted upon.
You will perform comprehensive reconciliations, resolve discrepancies, and provide evidence-based recommendations to optimise medication use while supporting secure discharge planning
